I authorize DNA Diagnostics Center ("the Company") to conduct Ancestry DNA testing with the submitted specimens. I understand that samples will be stored in the laboratory for 90 days after receipt, after which they will be destroyed according to laboratory procedures.

I acknowledge that this DNA testing is for recreational purposes only and is not for any medical purpose. I understand that test results do not provide statistically conclusive evidence of any biological relationship.

I acknowledge and understand that the Company shall not be liable for its inability to produce test results due to inadequate specimen. The Company may request additional samples for further testing.

I understand that there is no guaranteed turnaround time for test results and that the laboratory will make every effort to produce results within 4 weeks after payment and receipt of specimens in the laboratory.

I understand that results will be sent based on the delivery information provided on the order form as soon as results are available. It is my responsibility to provide delivery information that assures any privacy needs that I may have.

I understand that testing fees are non-refundable once I send DNA samples to the laboratory. If I change my mind about the test prior to the start of testing, I may notify the company in writing within 30 days of purchase to receive a refund minus a processing fee of $30.

I acknowledge and agree that the Company's liability to me arising out of or in any way related to the provision of testing services contemplated herein shall not exceed the cost of the test, and I agree to indemnify, defend, and hold the Company and its employees harmless from all further claims or damages.
