CONSENT FORM for TESTING
You are providing a biological sample for the purpose of BioGeographical
Ancestry (BGA) or other DNA analysis by DNAPrint® Genomics, Inc. You will have to sign a consent form at the time you send in your swabs to be tested. Because you
requested DNAPrint® Genomics Inc researchers to conduct a BGA analysis or other analysis from
your DNA, there are several things you should know about this test before
you consent:
Your BGA results will be your property. What you do with
them is up to you. The results are being provided to for your enjoyment,
and may or may not fulfill the requirements for legal admissibility.
If you have questions about the admissibility of this analysis for a specific
purpose, we advise you to consult with your counsel prior to ordering the
test.
The test results will be returned to you by U.S. standard
mail. Your information will be kept confidential, simi liar to medical records, however
they may be subpoenaed by court order, which DNAPrint® shall be required by
law to turn over the results of the tests to the proper authorities. You
are also aware that although expert testimony regarding these results will
be available through DNAPrint®, there is none provided with this test and
will only be available by separate agreement. (Please inquire)
This test is not for any medical purpose whatsoever. If
you are concerned about a potential genetic malady, you and your doctor might
choose to test specifically for it; this would require additional blood or
tissue samples and testing. You should discuss this option with your doctor
or genetic counselor. If you want to be tested, your doctor can refer you
for the appropriate genetic counseling and testing.
The risks associated with submitting DNA for this test are minimal. If you collect the sample incorrectly, discomfort and temporary bruising at the collection site is possible but no customers have experienced this to date. If you do not follow collection instructions properly, you could experience other risks such as possible infection, puncture, bleeding or swelling of surrounding tissue at the collection site.
The cheek swab collection kit you have ordered contains all the necessary instructions for collecting your DNA. Therefore, DNAPrint Genomics, Inc. cannot be held liable for consequences arising from the improper collection of your DNA.
These are the best-known risks and challenges of genetic
research. There might be other risks such as social
or psychological that are not yet know. Some people may be surprised by their results, and the
results may leave a lasting impression on how they view themselves, or perhaps
how others view them. If you are concerned about this possibility, we strongly
recommend you consult appropriate professionals before ordering the
test.
I understand that the ancestry tests provide statistical
estimates. I understand that the AncestryByDNA™ and EuroDNA™ test is not a black/white
litmus test, that my answer will be a range of likely percentages, and that
the most likely of all of these will be presented to me as the Most Likely
Estimate, and that in this regard, my answer will be much like a hurricane
track projection used by meteorologists (which is compiled from multiple
statistical estimates). I understand that while obtaining a single best answer
from a statistical test is possible, the prudent customer will consider their
answer to be a range of likely percentages, and to consider their Most Likely
Estimate to simply be those of this range that is the best estimate.
